A young adult in Nevada was confirmed to have been infected with the coronavirus twice, marking the first confirmed case of reinfection in the U.S., and the fourth in the world. This past week, researchers reported cases of reinfection in a patient in Hong Kong and two patients in Europe; but in all three cases, the patients either developed a milder form of COVID-19 or were asymptomatic the second time around, according to NBC News. But according to the new case study, the 25-year-old Nevada patient developed more severe symptoms the second time he was infected. His case was published as a preprint on the Social Science Research Network (SSRN) and hasn't yet been peer-reviewed. The researchers have submitted their paper to the journal The Lancet Infectious Diseases. "It is important to note, that this is a singular finding," study co-author Mark Pandori, the director of the Nevada State Public Health Laboratory said in a statement. "It does not provide any information to us with regard to the generalizability of this phenomenon." The 25-year-old first tested positive for COVID-19 in mid-April after he developed typical symptoms of the disease, including headache, cough, sore throat, nausea and diarrhea. Within 10 days, his symptoms had resolved and he later tested negative for the virus twice. But at the end of May, the patient developed a fever, headache, dizziness, cough, nausea and diarrhea once more. Within a week, his blood oxygen levels dropped and he was hospitalized, where he received oxygen support. New research published in the New England Journal of Medicinesuggests antibody levels plummet during the first three months after a person is infected, decreasing by half every 36 days. If this rate of antibody decay is sustained over time, the study authors said, antibodies would disappear within about a year. Earlier this month, a preliminary study from researchers in the UK showed that antibodies may last less than three months. Researchout of Spain has suggested an even shorter lifespan: three to five weeks in some patients. Combined, these findings have implications for vaccine development, since the efficacy of a vaccine hinges on how well a dose of weakened or dead virus prompts your body to generate antibodies that protect you from infection. If antibodies are fleeting, a vaccine's protection would be fleeting, too. . . . The UK study, meanwhile, suggested that antibodies could disappear completely within three months. The research tested more than 90 coronavirus patients for antibodies over 94 days. The results showed that three weeks after infection, just 60% of patients had developed a "potent" antibody response. In the weeks that followed, a mere 17% of those patients maintained that level of antibodies, The Guardian reported. In the other patients, antibody levels declined 23-fold or became undetectable within three months. Another study of patients in Wanzhou, China, published in June, found similar results. The researchers tested for antibodies in 37 people who had fallen ill and recovered, also testing 37 others who'd tested positive for the virus but never showed symptoms. About eight weeks after recovery, antibodies dropped to undetectable levels in 40% of the asymptomatic people and in 13% of those who had symptoms. Athena Akrami’s neuroscience lab reopened last month without her. Life for the 38-year-old is a pale shadow of what it was before 17 March, the day she first experienced symptoms of the novel coronavirus. At University College London (UCL), Akrami’s students probe how the brain organizes memories to support learning, but at home, she struggles to think clearly and battles joint and muscle pain. “I used to go to the gym three times a week,” Akrami says. Now, “My physical activity is bed to couch, maybe couch to kitchen.” Her early symptoms were textbook for COVID-19: a fever and cough, followed by shortness of breath, chest pain, and extreme fatigue. For weeks, she struggled to heal at home. But rather than ebb with time, Akrami’s symptoms waxed and waned without ever going away. She’s had just 3 weeks since March when her body temperature was normal. “Everybody talks about a binary situation, you either get it mild and recover quickly, or you get really sick and wind up in the ICU,” says Akrami, who falls into neither category. Thousands echo her story in online COVID-19 support groups. Outpatient clinics for survivors are springing up, and some are already overburdened. Akrami has been waiting more than 4 weeks to be seen at one of them, despite a referral from her general practitioner. The list of lingering maladies from COVID-19 is longer and more varied than most doctors could have imagined. Ongoing problems include fatigue, a racing heartbeat, shortness of breath, achy joints, foggy thinking, a persistent loss of sense of smell, and damage to the heart, lungs, kidneys, and brain. The likelihood of a patient developing persistent symptoms is hard to pin down because different studies track different outcomes and follow survivors for different lengths of time. One group in Italy found that 87% of a patient cohort hospitalized for acute COVID-19 was still struggling 2 months later. Data from the COVID Symptom Study, which uses an app into which millions of people in the United States, United Kingdom, and Sweden have tapped their symptoms, suggest 10% to 15% of people—including some “mild” cases—don’t quickly recover. But with the crisis just months old, no one knows how far into the future symptoms will endure, and whether COVID-19 will prompt the onset of chronic diseases. Researchers are now facing a familiar COVID-19 narrative: trying to make sense of a mystifying illness. Distinct features of the virus, including its propensity to cause widespread inflammation and blood clotting, could play a role in the assortment of concerns now surfacing. “We’re seeing a really complex group of ongoing symptoms,” says Rachael Evans, a pulmonologist at the University of Leicester. Survivor studies are starting to probe them. This month, researchers across the United Kingdom including Evans launched a study that will follow 10,000 survivors for 1 year to start, and up to 25 years. Ultimately, researchers hope not just to understand the disease’s long shadow, but also to predict who’s at highest risk of lingering symptoms and learn whether treatments in the acute phase of illness can head them off. For Götz Martin Richter, a radiologist at the Klinikum Stuttgart in Germany, what’s especially striking is that just as the illness’ acute symptoms vary unpredictably, so, too, do those that linger. Richter thinks of two patients he has treated: a middle-aged man who experienced mild pneumonia from COVID-19, and an elderly woman already suffering from chronic leukemia and arterial disease, who almost died from the virus and had to be resuscitated. Three months later, the man with the mild case “falls asleep all day long and cannot work,” Richter says. The woman has minimal lung damage and feels fine. EARLY IN the pandemic, doctors learned that SARS-CoV-2, the virus that causes COVID-19, can disrupt a breathtaking array of tissues in the body. Like a key fitting neatly into a lock, SARS-CoV-2 uses a spike protein on its surface to latch onto cells’ ACE2 receptors. The lungs, heart, gut, kidneys, blood vessels, and nervous system, among other tissues, carry ACE2 on their cells’ surfaces—and thus, are vulnerable to COVID-19. The virus can also induce a dramatic inflammatory reaction, including in the brain. Often, “The danger comes when the body responds out of proportion to the infection,” says Adrija Hajra, a physician at Albert Einstein College of Medicine in New York City. She continues to care for those who were infected in the spring and are still recovering. Despite the novelty of SARS-CoV-2, its long-term effects have precedents: Infections with other pathogens are associated with lasting impacts ranging from heart problems to chronic fatigue. “Medicine has been used to dealing with this problem” of acute viral illness followed by ongoing symptoms, says Michael Zandi, a neurologist at UCL. Even common illnesses such as pneumonia can mean a months long recovery. “I see a lot of people who had [the brain inflammation] encephalitis 3, 4 years ago, and still can’t think, or are tired,” Zandi says. Infections with certain bacteria and Zika virus, among others, are linked to Guillain-Barre syndrome, in which the immune system attacks nerve tissue, causing tingling, weakness, and paralysis. (Some cases of Guillain-Barre after COVID-19 have been reported, but “it’s not definite [there’s] a spike,” says Rachel Brown, a UCL neurologist who works with Zandi.) FOR DIAGRAM ON CONTINUED ILLNESSES - CLICKIE - DOWN ARTICLE AT THIS POINT Based on experience with other viruses, doctors can “extrapolate and anticipate” potential long-term effects of COVID-19, says Jeffrey Goldberger, chief of cardiology at the University of Miami. Like SARS-CoV-2, some other viruses, such as Epstein-Barr, can damage heart tissue, for example. In those infections, the organ sometimes heals completely. Sometimes, scarring is mild. “Or,” Goldberger says, “it could be severe and lead to heart failure.” Michael Marks, an infectious disease specialist at the London School of Hygiene & Tropical Medicine who’s helping lead the U.K. survivor study, says he’s not too surprised at emerging aftereffects. “What we’re experiencing is an epidemic of severe illness,” he says. “So therefore, there is an epidemic” of chronic illness that follows it. But outcomes following SARS-CoV-2 also appear distinct in ways both hopeful and dispiriting. Early this year, many doctors feared the virus would induce extensive, permanent lung damage in many survivors because two other coronaviruses, the viruses that cause the first severe acute respiratory syndrome (SARS) and Middle East respiratory syndrome, can devastate the lungs. One study of health care workers with SARS in 2003 found that those with lung lesions 1 year after infection still had them after 15 years. “We expected to see a lot of long-term damage from COVID-19: scarring, decreased lung function, decreased exercise capacity,” says Ali Gholamrezanezhad, a radiologist at the Keck School of Medicine at the University of Southern California who in mid-January began to review lung scans from COVID-19 patients in Asia. Hundreds of scans later, he has concluded that COVID-19 ravages the lungs less consistently and aggressively than SARS did, when about 20% of patients sustained lasting lung damage. “COVID-19 is in general a milder disease,” he says. At the same time, the sheer breadth of complications linked to COVID-19 is mind-boggling. In late April, Akrami collaborated with Body Politic, a group of COVID-19 survivors, to survey more than 600 who still had symptoms after 2 weeks. She logged 62 different symptoms and is now readying the findings for publication and developing a second survey to capture longer term ailments. “Even though it’s one virus, it can cause all different kinds of diseases in people,” says Akiko Iwasaki, an immunologist at Yale University who is studying lingering effects on the immune system. BY NOW IT’S CLEAR that many people with COVID-19 severe enough to put them in a hospital face a long recovery. The virus ravages the heart, for example, in multiple ways. Direct invasion of heart cells can damage or destroy them. Massive inflammation can affect cardiac function. The virus can blunt the function of ACE2 receptors, which normally help protect heart cells and degrade angiotensin II, a hormone that increases blood pressure. Stress on the body from fighting the virus can prompt release of adrenaline and epinephrine, which can also “have a deleterious effect on the heart,” says Raul Mitrani, a cardiac electrophysiologist at the University of Miami who collaborates with Goldberger. Mitrani and Goldberger, who co-authored a June paper in Heart Rhythm urging follow-up of patients who might have heart damage, worry in particular about the enzyme troponin, which is elevated in 20% to 30% of hospitalized COVID-19 patients and signifies cardiac damage. (Troponin is sky-high during a heart attack, for example.) How the heart heals following COVID-19 might determine whether an irregular heartbeat develops or persists, Goldberger believes. “We have one guy in the hospital right now who had COVID 2 months ago and had all sorts of arrhythmia problems” then, Goldberger says. “He’s recovered from his COVID, but still has the arrhythmia.” For some patients with coronavirus-induced heart problems, treatments as simple as cholesterol-lowering drugs, aspirin, or beta blockers could help, Goldberger says. Many people the pair has seen with heart complications post–COVID-19 had preexisting conditions, most commonly diabetes and hypertension. COVID-19, Goldberger suspects, tips them into more hazardous terrain or accelerates the onset of heart problems that, absent the coronavirus, might have developed later. But other patients are affected without apparent risk factors: A paper this week in JAMA Cardiology found that 78 of 100 people diagnosed with COVID-19 had cardiac abnormalities when their heart was imaged on average 10 weeks later, most often inflammation in heart muscle. Many of the participants in that study were previously healthy, and some even caught the virus while on ski trips, according to the authors. Severe lung scarring appears less common than feared—Gholamrezanezhad knows of only one recovered patient who still needs oxygen at rest. Scarring seems most likely to accompany underlying lung disease, hypertension, obesity, and other conditions. Lung damage is also seen in people who spend weeks on a ventilator. Gholamrezanezhad suspects that, as with harm to the heart, previously healthy people are not exempt from the virus’ long-term effects on the lungs, though their risk is likely lower. Then there’s the nervous system, a worrying target. Severe complications seem relatively rare but aren’t limited to those whom the virus renders critically ill. Brown, Zandi, and colleagues described 43 people with neurologic complications this month in Brain; many had been hospitalized during their acute infection, but not always for long—and for some, neurologic problems were their most debilitating symptom and the reason for hospital admission. Several were struggling to recover from encephalitis. Others had inflammation in their brain’s white matter, which helps transmit electrical signals. Separately, doctors are starting to see a class of patients who, like Akrami, struggle to think clearly—another outcome physicians have come upon in the past. After some severe viral infections, there are “those people who still don’t feel quite right afterward, but have normal brain scans,” Brown says. Some neurologists and patients describe the phenomenon as “brain fog.” It’s largely a mystery, though one theory suggests it’s similar to a “postviral fatigue related to inflammation in the body,” Brown says. Could that be happening here? “Who knows, really?” Brown asks. “These patients need to be followed.” PEOPLE LIKE THESE pose a growing concern (though they are also often dismissed by physicians). Collectively, these “long-haulers” describe dozens of symptoms, including many that could have multiple causes, such as fatigue, joint pain, and fever. “It’s time to give some voice to this huge population of patients,” Akrami says. The most bedeviling and common lingering symptom seems to be fatigue, but researchers caution against calling it chronic fatigue syndrome. That’s “a specific diagnosis,” Marks says. “You might have fibrosis in the lungs, and that will make you feel fatigued; you might have impaired heart function, and that will make you feel fatigued.” Trying to trace symptoms to their source is critical to understanding and ultimately managing them, he says. Iwasaki agrees. Doctors would treat symptoms differently depending on whether they result from a lingering infection or are rooted in autoimmune abnormalities. She has begun to recruit people who weren’t hospitalized when they had COVID-19 and will sift through her volunteers’ immune cells, examine whether they’re primed to attack, and measure whether the balance among different cell types is as it should be. She’ll also hunt for virus in saliva. “We’re pretty much searching for anything,” she says. Iwasaki is especially struck by the number of young, healthy, active people—people like Akrami—who fall into the long-hauler category. As she and others struggle to find ways to help them, she wonders what might head off their symptoms. One possibility, she says, is monoclonal antibodies, which are now being tested as a treatment for acute infection and might also forestall lasting immune problems. Hers is one of several survivor studies now underway. While Goldberger’s hometown of Miami faces a surge of acutely ill patients, he is looking ahead, applying for funding to image the heart and map its electrical activity in COVID-19 patients after they leave the hospital. Gholamrezanezhad is recruiting 100 patients after hospital discharge to follow for up to 2 years for lung assessments. Like many physicians, he fears the societal impact of even uncommon complications, including in the millions of people never hospitalized. “When you consider how many people are getting the disease, it’s a big problem,” he says. Across the Atlantic Ocean, Richter has recruited 300 volunteers in Germany for long-term follow-up, including lung scans. In the United Kingdom, patients will soon be able to sign up for that country’s survivor study, with many giving blood samples and being examined by specialists. The researchers will probe patients’ DNA and examine other characteristics such as age and health history to learn what might protect them from, or make them susceptible to, a range of COVID-19 induced health problems. Knowing who’s at risk of, say, kidney failure or cardiac arrhythmia could mean more targeted follow-up. The U.K. researchers are also keen to see whether patients who received certain treatments in the acute phase of illness, such as steroids or blood thinners, are less prone to later complications. For her part, Akrami is one of 2 million people infected weeks or months ago participating in the COVID Symptom Study. The study welcomes anyone infected, and with 10% to 15% of people who use the app reporting ongoing symptoms, it has already yielded a welter of data, says Andrew Chan, an epidemiologist and physician at Harvard Medical School. As he and his colleagues parse the data, they are identifying distinct “types” of acute illness, based on clusters of symptoms. Chan wonders whether certain early symptoms correlate with specific ones that linger. He acknowledges the risk that the app’s data could be skewed, because people who aren’t feeling well may be more likely to participate than those who have smooth recoveries. “We’re trying to develop data analysis tools” to account for that tilt, he says, “similar to methods used in polling. You have to weigh the biases.” One of the few systematic, long-term studies of COVID-19 patients with mild acute symptoms is underway in San Francisco, where researchers are recruiting 300 adults from local doctors and hospitals, for 2 years of follow-up. “We don’t have a broad idea of what’s happening” after the initial illness, says Steven Deeks, an HIV researcher at the University of California, San Francisco, who is leading the study, modeled on HIV cohorts he has followed for decades. What does “ongoing symptoms” even mean, Deeks asks. “Is that weeks, months? We don’t know that it’s years.” More than 100 people ranging in age from 18 to 80 have signed up so far. Cardiologists, neurologists, pulmonologists, and others are assessing the volunteers, and blood, saliva, and other biological specimens are being banked and analyzed. Although scientists hope they’ll learn how to avert chronic symptoms and help patients currently suffering, this latest chapter in the COVID-19 chronicle has been sobering. The message many researchers want to impart: Don’t underestimate the force of this virus. “Even if the story comes out a little scary, we need a bit of that right now,” Iwasaki says, because the world needs to know how high the stakes are. “Once the disease is established, it’s really hard to go backward.” How does coronavirus kill? LIKE MAGIC! “[The disease] can attack almost anything in the body with devastating consequences,” says cardiologist Harlan Krumholz of Yale University and Yale-New Haven Hospital, who is leading multiple efforts to gather clinical data on COVID-19. “Its ferocity is breathtaking and humbling.” Understanding the rampage could help the doctors on the front lines treat the fraction of infected people who become desperately and sometimes mysteriously ill. Does a dangerous, newly observed tendency to blood clotting transform some mild cases into life-threatening emergencies? Is an overzealous immune response behind the worst cases, suggesting treatment with immune-suppressing drugs could help? What explains the startlingly low blood oxygen that some physicians are reporting in patients who nonetheless are not gasping for breath? “Taking a systems approach may be beneficial as we start thinking about therapies,” says Nilam Mangalmurti, a pulmonary intensivist at the Hospital of the University of Pennsylvania (HUP). Related story What follows is a snapshot of the fast-evolving understanding of how the virus attacks cells around the body, especially in the roughly 5% of patients who become critically ill. . . . . . Despite the more than 1000 papers now spilling into journals and onto preprint servers every week, a clear picture is elusive, as the virus acts like no pathogen humanity has ever seen. . . . Without larger, prospective controlled studies that are only now being launched, scientists must pull information from small studies and case reports, often published at warp speed and not yet peer reviewed. “We need to keep a very open mind as this phenomenon goes forward,” says Nancy Reau, a liver transplant physician who has been treating COVID-19 patients at Rush University Medical Center. “We are still learning.” The infection begins When an infected person expels virus-laden droplets and someone else inhales them, the novel coronavirus, called SARS-CoV-2, enters the nose and throat. It finds a welcome home in the lining of the nose, according to a preprint from scientists at the Wellcome Sanger Institute and elsewhere. They found that cells there are rich in a cell-surface receptor called angiotensin-converting enzyme 2 (ACE2). Throughout the body, the presence of ACE2, which normally helps regulate blood pressure, marks tissues vulnerable to infection, because the virus requires that receptor to enter a cell. Once inside, the virus hijacks the cell’s machinery, making myriad copies of itself and invading new cells. As the virus multiplies, an infected person may shed copious amounts of it, especially during the first week or so. Symptoms may be absent at this point. Or the virus’ new victim may develop a fever, dry cough, sore throat, loss of smell and taste, or head and body aches. If the immune system doesn’t beat back SARS-CoV-2 during this initial phase, the virus then marches down the windpipe to attack the lungs, where it can turn deadly. The thinner, distant branches of the lung’s respiratory tree end in tiny air sacs called alveoli, each lined by a single layer of cells that are also rich in ACE2 receptors. Normally, oxygen crosses the alveoli into the capillaries, tiny blood vessels that lie beside the air sacs; the oxygen is then carried to the rest of the body. But as the immune system wars with the invader, the battle itself disrupts this healthy oxygen transfer. Front-line white blood cells release inflammatory molecules called chemokines, which in turn summon more immune cells that target and kill virus-infected cells, leaving a stew of fluid and dead cells—pus—behind. This is the underlying pathology of pneumonia, with its corresponding symptoms: coughing; fever; and rapid, shallow respiration (see graphic). Some COVID-19 patients recover, sometimes with no more support than oxygen breathed in through nasal prongs. But others deteriorate, often quite suddenly, developing a condition called acute respiratory distress syndrome (ARDS). Oxygen levels in their blood plummet and they struggle ever harder to breathe. On x-rays and computed tomography scans, their lungs are riddled with white opacities where black space—air—should be. Commonly, these patients end up on ventilators. Many die. Autopsies show their alveoli became stuffed with fluid, white blood cells, mucus, and the detritus of destroyed lung cells. An invader’s impactIn serious cases, SARS-CoV-2 lands in the lungs and can do deep damage there. But the virus, or the body’s response to it, can injure many other organs. Scientists are just beginning to probe the scope and nature of that harm. Click on organ name for more. Patients cough, fevers rise, and breathing becomes labored. CLICK FOR ARTICLE LINK - TABLES & FIGURES Some clinicians suspect the driving force in many gravely ill patients’ downhill trajectories is a disastrous overreaction of the immune system known as a “cytokine storm,” which other viral infections are known to trigger. Cytokines are chemical signaling molecules that guide a healthy immune response; but in a cytokine storm, levels of certain cytokines soar far beyond what’s needed, and immune cells start to attack healthy tissues. Blood vessels leak, blood pressure drops, clots form, and catastrophic organ failure can ensue. Some studies have shown elevated levels of these inflammation-inducing cytokines in the blood of hospitalized COVID-19 patients. “The real morbidity and mortality of this disease is probably driven by this out of proportion inflammatory response to the virus,” says Jamie Garfield, a pulmonologist who cares for COVID-19 patients at Temple University Hospital. But others aren’t convinced. “There seems to have been a quick move to associate COVID-19 with these hyperinflammatory states. I haven’t really seen convincing data that that is the case,” says Joseph Levitt, a pulmonary critical care physician at the Stanford University School of Medicine. He’s also worried that efforts to dampen a cytokine response could backfire. Several drugs targeting specific cytokines are in clinical trials in COVID-19 patients. But Levitt fears those drugs may suppress the immune response that the body needs to fight off the virus. “There’s a real risk that we allow more viral replication,” Levitt says. Meanwhile, other scientists are zeroing in on an entirely different organ system that they say is driving some patients’ rapid deterioration: the heart and blood vessels. Striking the heart In Brescia, Italy, a 53-year-old woman walked into the emergency room of her local hospital with all the classic symptoms of a heart attack, including telltale signs in her electrocardiogram and high levels of a blood marker suggesting damaged cardiac muscles. Further tests showed cardiac swelling and scarring, and a left ventricle—normally the powerhouse chamber of the heart—so weak that it could only pump one-third its normal amount of blood. But when doctors injected dye in the coronary arteries, looking for the blockage that signifies a heart attack, they found none. Another test revealed why: The woman had COVID-19. How the virus attacks the heart and blood vessels is a mystery, but dozens of preprints and papers attest that such damage is common. A 25 March paper in JAMA Cardiology documented heart damage in nearly 20% of patients out of 416 hospitalized for COVID-19 in Wuhan, China. In another Wuhan study, 44% of 36 patients admitted to the ICU had arrhythmias. The disruption seems to extend to the blood itself. Among 184 COVID-19 patients in a Dutch ICU, 38% had blood that clotted abnormally, and almost one-third already had clots, according to a 10 April paper in Thrombosis Research. Blood clots can break apart and land in the lungs, blocking vital arteries—a condition known as pulmonary embolism, which has reportedly killed COVID-19 patients. Clots from arteries can also lodge in the brain, causing stroke. Many patients have “dramatically” high levels of D-dimer, a byproduct of blood clots, says Behnood Bikdeli, a cardiovascular medicine fellow at Columbia University Medical Center. “The more we look, the more likely it becomes that blood clots are a major player in the disease severity and mortality from COVID-19,” Bikdeli says. Infection may also lead to blood vessel constriction. Reports are emerging of ischemia in the fingers and toes—a reduction in blood flow that can lead to swollen, painful digits and tissue death. Behnood Bikdeli, Columbia University Irving Medical Center In the lungs, blood vessel constriction might help explain anecdotal reports of a perplexing phenomenon seen in pneumonia caused by COVID-19: Some patients have extremely low blood-oxygen levels and yet are not gasping for breath. It’s possible that at some stages of disease, the virus alters the delicate balance of hormones that help regulate blood pressure and constricts blood vessels going to the lungs. So oxygen uptake is impeded by constricted blood vessels, rather than by clogged alveoli. “One theory is that the virus affects the vascular biology and that’s why we see these really low oxygen levels,” Levitt says. If COVID-19 targets blood vessels, that could also help explain why patients with pre-existing damage to those vessels, for example from diabetes and high blood pressure, face higher risk of serious disease. Recent Centers for Disease Control and Prevention (CDC) data on hospitalized patients in 14 U.S. states found that about one-third had chronic lung disease—but nearly as many had diabetes, and fully half had pre-existing high blood pressure. Mangalmurti says she has been “shocked by the fact that we don’t have a huge number of asthmatics” or patients with other respiratory diseases in HUP’s ICU. “It’s very striking to us that risk factors seem to be vascular: diabetes, obesity, age, hypertension.” Scientists are struggling to understand exactly what causes the cardiovascular damage. The virus may directly attack the lining of the heart and blood vessels, which, like the nose and alveoli, are rich in ACE2 receptors. Or perhaps lack of oxygen, due to the chaos in the lungs, damages blood vessels. Or a cytokine storm could ravage the heart as it does other organs. “We’re still at the beginning,” Krumholz says. “We really don’t understand who is vulnerable, why some people are affected so severely, why it comes on so rapidly … and why it is so hard [for some] to recover.” Multiple Battlefields The worldwide fears of ventilator shortages for failing lungs have received plenty of attention. Not so a scramble for another type of equipment: dialysis machines. “If these folks are not dying of lung failure, they’re dying of renal failure,” says neurologist Jennifer Frontera of New York University’s Langone Medical Center, which has treated thousands of COVID-19 patients. Her hospital is developing a dialysis protocol with different machines to support additional patients. The need for dialysis may be because the kidneys, abundantly endowed with ACE2 receptors, present another viral target. According to one preprint, 27% of 85 hospitalized patients in Wuhan had kidney failure. Another reported that 59% of nearly 200 hospitalized COVID-19 patients in China’s Hubei and Sichuan provinces had protein in their urine, and 44% had blood; both suggest kidney damage. Those with acute kidney injury (AKI), were more than five times as likely to die as COVID-19 patients without it, the same Chinese preprint reported. “The lung is the primary battle zone. But a fraction of the virus possibly attacks the kidney. And as on the real battlefield, if two places are being attacked at the same time, each place gets worse,” says Hongbo Jia, a neuroscientist at the Chinese Academy of Sciences’s Suzhou Institute of Biomedical Engineering and Technology and a co-author of that study. Viral particles were identified in electron micrographs of kidneys from autopsies in one study, suggesting a direct viral attack. But kidney injury may also be collateral damage. Ventilators boost the risk of kidney damage, as do antiviral compounds including remdesivir, which is being deployed experimentally in COVID-19 patients. Cytokine storms also can dramatically reduce blood flow to the kidney, causing often-fatal damage. And pre-existing diseases like diabetes can increase the chances of kidney injury. “There is a whole bucket of people who already have some chronic kidney disease who are at higher risk for acute kidney injury,” says Suzanne Watnick, chief medical officer at Northwest Kidney Centers. Buffeting the brain Another striking set of symptoms in COVID-19 patients centers on the brain and central nervous system. Frontera says neurologists are needed to assess 5% to 10% of coronavirus patients at her hospital. But she says that “is probably a gross underestimate” of the number whose brains are struggling, especially because many are sedated and on ventilators. Frontera has seen patients with the brain inflammation encephalitis, with seizures, and with a “sympathetic storm,” a hyperreaction of the sympathetic nervous system that causes seizurelike symptoms and is most common after a traumatic brain injury. Some people with COVID-19 briefly lose consciousness. Others have strokes. Many report losing their sense of smell. And Frontera and others wonder whether in some cases, infection depresses the brain stem reflex that senses oxygen starvation. This is another explanation for anecdotal observations that some patients aren’t gasping for air, despite dangerously low blood oxygen levels. ACE2 receptors are present in the neural cortex and brain stem, says Robert Stevens, an intensive care physician at Johns Hopkins Medicine. But it’s not known under what circumstances the virus penetrates the brain and interacts with these receptors. That said, the coronavirus behind the 2003 severe acute respiratory syndrome (SARS) epidemic—a close cousin of today’s culprit—could infiltrate neurons and sometimes caused encephalitis. On 3 April, a case study in the International Journal of Infectious Diseases, from a team in Japan, reported traces of new coronavirus in the cerebrospinal fluid of a COVID-19 patient who developed meningitis and encephalitis, suggesting it, too, can penetrate the central nervous system. But other factors could be damaging the brain. For example, a cytokine storm could cause brain swelling, and the blood’s exaggerated tendency to clot could trigger strokes. The challenge now is to shift from conjecture to confidence, at a time when staff are focused on saving lives, and even neurologic assessments like inducing the gag reflex or transporting patients for brain scans risk spreading the virus. Last month, Sherry Chou, a neurologist at the University of Pittsburgh Medical Center, began to organize a worldwide consortium that now includes 50 centers to draw neurological data from care patients already receive. The early goals are simple: Identify the prevalence of neurologic complications in hospitalized patients and document how they fare. Longer term, Chou and her colleagues hope to gather scans, lab tests, and other data to better understand the virus’ impact on the nervous system, including the brain. Chou speculates about a possible invasion route: through the nose, then upward and through the olfactory bulb—explaining reports of a loss of smell—which connects to the brain. “It’s a nice sounding theory,” she says. “We really have to go and prove that.” Most neurological symptoms “are reported from colleague to colleague by word of mouth,” Chou adds. “I don’t think anybody, and certainly not me, can say we’re experts.” Reaching the gutIn early March, a 71-year-old Michigan woman returned from a Nile River cruise with bloody diarrhea, vomiting, and abdominal pain. Initially doctors suspected she had a common stomach bug, such as Salmonella. But after she developed a cough, doctors took a nasal swab and found her positive for the novel coronavirus. A stool sample positive for viral RNA, as well as signs of colon injury seen in an endoscopy, pointed to a gastrointestinal (GI) infection with the coronavirus, according to a paper posted online in The American Journal of Gastroenterology (AJG). Her case adds to a growing body of evidence suggesting the new coronavirus, like its cousin SARS, can infect the lining of the lower digestive tract, where the crucial ACE2 receptors are abundant. Viral RNA has been found in as many as 53% of sampled patients’ stool samples. And in a paper in press at Gastroenterology, a Chinese team reported finding the virus’ protein shell in gastric, duodenal, and rectal cells in biopsies from a COVID-19 patient. “I think it probably does replicate in the gastrointestinal tract,” says Mary Estes, a virologist at Baylor College of Medicine. Recent reports suggest up to half of patients, averaging about 20% across studies, experience diarrhea, says Brennan Spiegel of Cedars-Sinai Medical Center in Los Angeles, co–editor-in-chief of AJG. GI symptoms aren’t on CDC’s list of COVID-19 symptoms, which could cause some COVID-19 cases to go undetected, Spiegel and others say. “If you mainly have fever and diarrhea, you won’t be tested for COVID,” says Douglas Corley of Kaiser Permanente, Northern California, co-editor of Gastroenterology. The presence of virus in the GI tract raises the unsettling possibility that it could be passed on through feces. But it’s not yet clear whether stool contains intact, infectious virus, or only RNA and proteins. To date, “We have no evidence” that fecal transmission is important, says coronavirus expert Stanley Perlman of the University of Iowa. CDC says that based on experiences with SARS and with the virus that causes Middle East respiratory syndrome, another dangerous cousin of the new coronavirus, the risk from fecal transmission is probably low. The intestines are not the end of the disease’s march through the body. For example, up to one-third of hospitalized patients develop conjunctivitis—pink, watery eyes—although it’s not clear that the virus directly invades the eye. Other reports suggest liver damage: More than half of COVID-19 patients hospitalized in two Chinese centers had elevated levels of enzymes indicating injury to the liver or bile ducts. But several experts told Science that direct viral invasion isn’t likely the culprit. They say other events in a failing body, like drugs or an immune system in overdrive, are more likely driving the liver damage. This map of the devastation that COVID-19 can inflict on the body is still just a sketch. It will take years of painstaking research to sharpen the picture of its reach, and the cascade of cardiovascular and immune effects it might set in motion. Dr. Fauci Backed Controversial Wuhan Lab with U.S. Dollars for Risky Coronavirus Research BY FRED GUTERL ON 4/28/20 AT 2:57 PM EDT Dr. Anthony Fauci is an adviser to President Donald Trump and something of an American folk hero for his steady, calm leadership during the pandemic crisis. At least one poll shows that Americans trust Fauci more than Trump on the coronavirus pandemic—and few scientists are portrayed on TV by Brad Pitt. But just last year, the National Institute for Allergy and Infectious Diseases, the organization led by Dr. Fauci, funded scientists at the Wuhan Institute of Virology and other institutions for work on gain-of-function research on bat coronaviruses. In 2019, with the backing of NIAID, the National Institutes of Health committed $3.7 million over six years for research that included some gain-of-function work. The program followed another $3.7 million, 5-year project for collecting and studying bat coronaviruses, which ended in 2019, bringing the total to $7.4 million. Many scientists have criticized gain of function research, which involves manipulating viruses in the lab to explore their potential for infecting humans, because it creates a risk of starting a pandemic from accidental release. SARS-CoV-2 , the virus now causing a global pandemic, is believed to have originated in bats. U.S. intelligence, after originally asserting that the coronavirus had occurred naturally, conceded last month that the pandemic may have originated in a leak from the Wuhan lab. (At this point most scientists say it's possible—but not likely—that the pandemic virus was engineered or manipulated.) Dr. Fauci did not respond to Newsweek's requests for comment. NIH responded with a statement that said in part: "Most emerging human viruses come from wildlife, and these represent a significant threat to public health and biosecurity in the US and globally, as demonstrated by the SARS epidemic of 2002-03, and the current COVID-19 pandemic.... scientific research indicates that there is no evidence that suggests the virus was created in a laboratory." The NIH research consisted of two parts. The first part began in 2014 and involved surveillance of bat coronaviruses, and had a budget of $3.7 million. The program funded Shi Zheng-Li, a virologist at the Wuhan lab, and other researchers to investigate and catalogue bat coronaviruses in the wild. This part of the project was completed in 2019. A second phase of the project, beginning that year, included additional surveillance work but also gain-of-function research for the purpose of understanding how bat coronaviruses could mutate to attack humans. The project was run by EcoHealth Alliance, a non-profit research group, under the direction of President Peter Daszak, an expert on disease ecology. NIH canceled the project just this past Friday, April 24th, Politico reported. Daszak did not immediately respond to Newsweek requests for comment. The project proposal states: "We will use S protein sequence data, infectious clone technology, in vitro and in vivo infection experiments and analysis of receptor binding to test the hypothesis that % divergence thresholds in S protein sequences predict spillover potential." In layman's terms, "spillover potential" refers to the ability of a virus to jump from animals to humans, which requires that the virus be able to attach to receptors in the cells of humans. SARS-CoV-2, for instance, is adept at binding to the ACE2 receptor in human lungs and other organs. According to Richard Ebright, an infectious disease expert at Rutgers University, the project description refers to experiments that would enhance the ability of bat coronavirus to infect human cells and laboratory animals using techniques of genetic engineering. In the wake of the pandemic, that is a noteworthy detail. Ebright, along with many other scientists, has been a vocal opponent of gain-of-function research because of the risk it presents of creating a pandemic through accidental release from a lab. Dr. Fauci is renowned for his work on the HIV/AIDS crisis in the 1990s. Born in Brooklyn, he graduated first in his class from Cornell University Medical College in 1966. As head of NIAID since 1984, he has served as an adviser to every U.S. president since Ronald Reagan. A decade ago, during a controversy over gain-of-function research on bird-flu viruses, Dr. Fauci played an important role in promoting the work. He argued that the research was worth the risk it entailed because it enables scientists to make preparations, such as investigating possible anti-viral medications, that could be useful if and when a pandemic occurred. The work in question was a type of gain-of-function research that involved taking wild viruses and passing them through live animals until they mutate into a form that could pose a pandemic threat. Scientists used it to take a virus that was poorly transmitted among humans and make it into one that was highly transmissible—a hallmark of a pandemic virus. This work was done by infecting a series of ferrets, allowing the virus to mutate until a ferret that hadn't been deliberately infected contracted the disease. The work entailed risks that worried even seasoned researchers. More than 200 scientists called for the work to be halted. The problem, they said, is that it increased the likelihood that a pandemic would occur through a laboratory accident. Dr. Fauci defended the work. "[D]etermining the molecular Achilles' heel of these viruses can allow scientists to identify novel antiviral drug targets that could be used to prevent infection in those at risk or to better treat those who become infected," wrote Fauci and two co-authors in the Washington Post on December 30, 2011. "Decades of experience tells us that disseminating information gained through biomedical research to legitimate scientists and health officials provides a critical foundation for generating appropriate countermeasures and, ultimately, protecting the public health." Nevertheless, in 2014, under pressure from the Obama administration, the National of Institutes of Health instituted a moratorium on the work, suspending 21 studies. Three years later, though—in December 2017—the NIH ended the moratorium and the second phase of the NIAID project, which included the gain-of-function research, began. The NIH established a framework for determining how the research would go forward: scientists have to get approval from a panel of experts, who would decide whether the risks were justified. The reviews were indeed conducted—but in secret, for which the NIH has drawn criticism. In early 2019, after a reporter for Science magazine discovered that the NIH had approved two influenza research projects that used gain of function methods, scientists who oppose this kind of research excoriated the NIH in an editorial in the Washington Post. "We have serious doubts about whether these experiments should be conducted at all," wrote Tom Inglesby of Johns Hopkins University and Marc Lipsitch of Harvard. "[W]ith deliberations kept behind closed doors, none of us will have the opportunity to understand how the government arrived at these decisions or to judge the rigor and integrity of that process." Reports: Trump Sabotaged Coronavirus Testing to Keep Numbers Low By Jonathan Chait AUG. 26, 2020 Public-health officials believe that widespread testing is a key element of any response to the coronavirus. President Trump does not believe this. And now the Centers for Disease Control and Prevention has officially adopted Trump’s view. It has modified its official guidelines, and no longer recommends that those who have been exposed to the virus but currently lack symptoms get a test. The first reports of this change were filled with comments from incredulous experts, whose assessments included “bizarre,” “very strange,” “this is going to make things worse,” and so on. Now we have an explanation for this bizarre policy: High-level officials ordered it. CNN reports the change “came this week as a result of pressure from the upper ranks of the Trump administration.” The New York Times adds, “One official said the directive came from the top down. Another said the guidelines were not written by the C.D.C. but were forced down.” Evidence that Trump has sought to slow down testing has been available for a long time. In March, Trump told reporters he kept infected passengers offshore on a cruise ship in order to hold down the official numbers of infections: “I like the numbers being where they are. I don’t need to have the numbers double because of one ship.” He has said for months that the availability of testing increases recorded case levels and makes him look bad. (“When you test, you have a case. When you test, you find something is wrong with people. If we didn’t do any testing, we would have very few cases.”) And he has insisted that many people who get tests don’t need them, because they’re young and will recover. “Many of those cases are young people that would heal in a day,” he said on July 19. “They have the sniffles and we put it down as a test.” In July, Trump announced at a rally, “You know testing is a double-edged sword … Here’s the bad part: When you do testing to that extent, you’re going to find more people; you’re going to find more cases. So I said to my people, ‘Slow the testing down please.’” His defenders insisted he was kidding. A reporter offered Trump the chance to make this defense. He did not. When asked, “But did you ask to slow [testing] down?” Trump shrugged and replied, “If it did slow down, frankly, I think we’re way ahead of ourselves, if you want to know the truth. The state reported that without the Quest results, the positivity rate for new cases totaling 3,773 for Aug. 31 is 5.9%. With the Quest results, that total rises to 7,643 cases with a positivity rate of 6.8%. The state said the majority of the Quest results were for tests more than two weeks old, and some as old as five months. DeSantis stressed that “the individuals who tested positive at that time were notified by the lab, which is obviously very important. The lab just did not submit those results to the state. ... That’s not acceptable.” He said the biggest impact of the mistake was that testing numbers play a huge role in determining public strategy. “These labs know that their results are being used by people to determine the course of certain policies,” he said. “Someone will say, ’the positive rate has gone up, maybe we can’t do in-person schooling.’ ... Right now, sea surface temperatures range anywhere from 70° to 80°+, which is a good two to four degrees above normal. But how does this warmer than normal water temperature impact us here locally? The hypothesis is that strong to severe thunderstorms in parts of the Deep South can be influence by the mild water. When temperatures are higher in the Gulf, there is more energy available for thunderstorms, hence you have an ample supply of moisture and instability needed for strong to severe storms to develop. There are six widely accepted conditions for hurricane development: 1. The first condition is that ocean waters must be above 26 degrees Celsius (79 degrees Fahrenheit). Below this threshold temperature, hurricanes will not form or will weaken rapidly once they move over water below this threshold. Ocean temperatures in the tropical East Pacific and the tropical Atlantic routinely surpass this threshold. 2. The second ingredient is distance from the equator. Without the spin of the earth and the resulting Corioles force, hurricanes would not form. Since the Corioles force is at a maximum at the poles and a minimum at the equator, hurricanes can not form within 5 degrees latitude of the equator. The Corioles force generates a counterclockwise spin to low pressure in the Northern Hemisphere and a clockwise spin to low pressure in the Southern Hemisphere. 3. The third ingredient is that of a saturated lapse rate gradient near the center of rotation of the storm. A saturated lapse rate insures latent heat will be released at a maximum rate. Hurricanes are warm core storms. The heat hurricanes generate is from the condensation of water vapor as it convectively rises around the eye wall. The lapse rate must be unstable around the eyewall to insure rising parcels of air will continue to rise and condense water vapor. 4. The fourth and one of the most important ingredients is that of a low vertical wind shear, especially in the upper level of the atmosphere. Wind shear is a change in wind speed with height. Strong upper level winds destroy the storms structure by displacing the warm temperatures above the eye and limiting the vertical accent of air parcels. Hurricanes will not form when the upper level winds are too strong. 5. The fifth ingredient is high relative humidity values from the surface to the mid levels of the atmosphere. Dry air in the mid levels of the atmosphere impedes hurricane development in two ways. First, dry air causes evaporation of liquid water. Since evaporation is a cooling process, it reduces the warm core structure of the hurricane and limits vertical development of convection. Second, dry air in the mid levels can create what is known as a trade wind inversion. This inversion is similar to sinking air in a high pressure system. The trade wind inversion produces a layer of warm temperatures and dryness in the mid levels of the atmosphere due to the sinking and adiabatic warming of the mid level air. This inhibits deep convection and produces a stable lapse rate. 6. The sixth ingredient is that of a tropical wave. Often hurricanes in the Atlantic begin as a thunderstorm complex that moves off the coast of Africa. It becomes what is known as a midtropospheric wave. If this wave encounters favorable conditions such as stated in the first five ingredients, it will amplify and evolve into a tropical storm or hurricane.
